Penyakit jamur kulit (DERMATOMIKOSIS)

                 Dermatomikosis adalah penyakit jamur yang menyerang kulit dapat berupa dermatofitosis dan non dermatofitosis, sering menjadi diagnosis banding daa menegakkan diaposis dermatis . Dermatofitosis disebabkan oleh golongan jamur dermatofit menyerang epidermis bagian supertisiaiis, kuku dan rambut. Ditandai oleh bercak eritemosa yang  berbatas tegas dalam skuama diatasnya kadang dijumpai papul dan vesikel kecil dibagian tepinya. Proses peradangan tepi lesi relatif lebih nyata dari pada bagian tengah, pada lesi yang menahun sering berupa bercak  hitam sedikit sisik. Non dermatofitosis disebabkan oleh Malassezia furfur, ditandai oleh bercak skuma halus yang berwarna putih sampai coklat hitam terutama di badan kadang meluas sampai keseluruh bagian tubuh. Pengobatan dapat dilakukan dengan dua cara yaitu:

1.       Secara topikal

Pengobatan topikal dapat menggunakan salep whitfield sehari 2-4 kali, salap 2-4 sehari 2 kali, slap miconazole sehari 2 kali. Pengobatan ini umumnya dilakukan minimal selama 3 minggu yaitu 2 minggu setelah tanda klinis membaik untuk mencegah kekambuhan pada obat fungistatik.

2.       Secara sistemik

Penggunaan obat sistemik tergantung obat oral yang di gunakan, lokasi dan penyebabnya. Pengobatan sistemik dapat menggunakan obat-obat anti jamur dengan lama pengobatan untuk oral Fungisidal 1-2 minggu dan untuk fungistatik 2-4 minggu. Berikut contah obat untuk anti jamur :

a.       Griseofulvin

Dihasilkan oleh penisilinum griseofulvinum, berkhasiat fungistatik pada penggunaan oral terhadap banyak dermatofit, efektif untuk mengobati infeksi kulit dan kuku menahun penyembuhan berlangsung sangat perlahan.

Indikasi                 : Infeksi dermatofitosis kulit, kulit kepala, rambut, kuku bila terapi topikal gagal

Kontra Indikasi  : Gangguan fungsi hati, kehamilan

Efek samping     : Sakit kepala, muntah

Sediaan                : Griseofulvin (generik ) tablet 125 mg

Spesialite obat  :Fulcin (pabrik  Zeneca ), Grivin (pabrik Pharos)

b.      Nistatin

Berasal dari Streptomyces moursei

Indikasi                 : Kandidiasis (stomatitis, sariawan pada mulut, vaginitis pada vagina)

Kontra indikasi  : -

Efek samping     : Mual, muntah, diare (diberikan peroral), iritasi lokal pada pemakaian topikal

Sediaan                                : Nistatin (generik) tablet 500.000 UI

Spesialite obat  : Candistatin (pabrik Pharos), flagystatin (pabrik Rhone Poulenc), Mycostatin (Squiib Indonesia)

c.       Amfoterisin B

Dihasilkan oleh Stertomyces nodosus

Indikasi                 : Kandidiasis intestinal

Kontra indikasi  : -

Efek samping     : Anoreksia, mual, muntah, diare, sakit kepala

Spesialite obat  : Amphotec (pabrik Zeneca Indonesia), Fungizone (pabrik Squiib indonesia)
